The fault handbook currently contains three chapters:

• system failures: lessons learned from medical device systems which have failed; a supporting paper Lessons from 342 Failures of Medical Devices provides details;
• classification of issues in C++, with guidance, and
• classification of OO faults, contributed by Donald G. Firesmith.

Over time, this handbook will contain additional chapters for other languages and domains. When additional data on failures found by users of systems are collected, new chapters will be added to address lessons learned. Once the public database contains data from many projects, a chapter of the handbook will contain fault frequency profiles derived from that data.
